Cedar Family Program provides support to families of children with special healthcare needs. Cedar staff work closely with families to complete a needs assessment, develop a plan with family-driven goals, and create a coordinated network of support for the family. These services benefit families who require intensive care management and coordination services related to behavioral health, social development, and/or medical intervention.
CEDAR is available statewide.
Interested parties may self-refer, we also accept referrals from primary care physicians, a health plan, schools, or other provider.
