Family Service of Rhode Island’s Healthy Transitions program helps young people ages 16–25 build the skills, support, and confidence they need to navigate the shift from adolescence to adulthood. Through individualized care and peer support, Healthy Transitions empowers participants to set personal goals, strengthen relationships, and access resources that promote independence, education, employment, and wellness.
Whether it’s managing stress, exploring career paths, or connecting to mental health services, our team walks alongside young people during this pivotal time, ensuring they feel supported and capable of creating a healthy, fulfilling future.
